Registering as a New Patient

The doctors welcome new patients who live within our practice area.

PLEASE NOTE: we require all forms to be returned, if any are missing we will not be able to register you with our surgery. If you have trouble completing a form please contact the surgery between 11am and 3pm (we tend to be quieter then) to speak with one of our team members. 

You may be asked to provide evidence of residency. Please bring with you a piece of photographic ID and confirmation of the address. As no one is automatically registered with our surgery, it is important that both you and all members of your household register at the same time. We recognise the important role of carers in looking after dependent relatives; please make it known if you are caring for someone. Who will be your doctor?

Your registration will be with the practice however you will be given a named doctor responsible for care however you can see any of the doctors practicing at the surgery, although you may have to wait longer to see a specific doctor. For continuity of care, we would encourage you to consult the same doctor as far as possible.

Summary Care Record

When registering with the practice you will also be asked whether you consent to your information being stored electronically and uploaded to a national database. This Summary Care Record is an electronic record which contains information about the medicines you take, allergies you suffer from and any bad reactions to medicines you have had.

Having this information stored in one place makes it easier for healthcare staff to treat you in an emergency wherever you are, or when your GP practice is closed. Strand Medical supports the use of Summary Care Records and so if you do not complete the form below and hand it in, we will assume consent.

Below are the forms required and the information needed to register with our practice.

1. Accessibility, ID and PPG info for adult patients – for information

3. GMS1 New Patient Form – to be completed with the below form

4. New Patient Questionnaire (adult) – to be completed

5. GMS3 Temporary Patient Registration Form

6. New Patient Questionnaire (16yrs and under) – to be completed

7. Overall questionnaire for new patients – to be completed

8. Summary Care Record letter – for information

9. Using-the-NHS-Information-for-visitors-from-abroad

Once these forms are filled out they should be returned to the surgery along with your proof of address and photographic ID.
