The doctors welcome new patients who live within our practice area.
Please note – we will still need to manually register you on are clinical system, therefore completing this online form does not mean you are immediately registered. However, we will endeavour to help you however we can.
(If you are registering as a temporary patient, please print and fill out the GMS3 and the Temporary Resident Questionnaire and return them to the surgery).

Named GP
The Practice now provides every patient with a named GP who will have overall responsibility for the care and support that our surgery provides to you. This does not prevent you from seeing any GP in the Practice as you currently do. Please do bear in mind that your named Doctor will not always be available to see or speak to you, you may be seen by or speak to an alternative Doctor.
If you would like to know the name of your accountable GP, please ask at the Reception desk or alternatively call the surgery on 01903 243351 in afternoon hours only.
Access to Medical Records
The practice complies with the GDPR Regulations brought in on 25th May 2018. Patients are able to access their medical records online or via our Medical Secretary team.
Please note: requests must be made in writing and can take up to 30 days to complete. If you have requested paper copies, we will inform you when they are ready for collection. We do not post patients records as they must be collected in person, please bring along ID for verification purposes.
